Please use this form to sign up for the Wednesday, June 4, sports physical exams hosted by Sacred Heart Cathedral Sports Medicine.
Please read through the instructions carefully:
1) Students must be enrolled on Magnus Health for the 2025-2026 school year prior to the sports physical appointment.
2) Magnus registration for the 2025-26 school year will be available on Sunday, June 1. Please log into your Magnus account and update your data as necessary. Please DO NOT register prior to June 1.
3) Enrolling on Magnus Health will allow parents to download a blank copy of the SHC Pre-participation Physical Exam (PPE) form. ONLY the SHC PPE form will be accepted. Please bring the SHC PPE form with you for the physicians to complete during your exam.
4) Registration for appointments will be open until full.
